Fill out and print the Visa Application Form on A4-size paper.
Submit all complete requirements to Travel Port Travel & Tours and settle the visa processing fee and visa fee.
Your application will be lodged, and results will be collected on your behalf.
Visa results are typically released after a minimum of 81 working days (processing time may vary).

General Requirements

Duly Accomplished Draft Application Form (you must answer every question, if not applicable, write N/A)
Duly Accomplished Form IMM 5476 (Use of Representative)
Original Valid Passport (6 months valid) and old passport with valid US visa, if applicable
Original Bank Certificate (Must include account type, current balance, account opening date, 6 months average daily balance)
Original Bank Statement or Certified True Copy of Passbook for the last 6 months
Any additional Relevant Documentation (last 2 pay slips, property titles, proof of pension, investments, etc.)
Copy of Recent Personal Income Tax Return
Hotel Booking Confirmation (if applicable)
Roundtrip Airline Ticket Reservation (if applicable)
Detailed itinerary during your trip (if applicable)
